WITHDRAWAL CARD NOTICE

If you have been laid off, terminated, quit, are on sick leave, or have retired from your Teamster employment, BE SURE TO REQUEST A WITHDRAWAL CARD from your Union Office. You may either call, stop in or write to the Union Office. Your dues must be paid for the last month in which you worked. Be sure to include your Social Security number and your last date of work.
When retiring, you will need a withdrawal card to remain in good standing. Withdrawal cards are no cost to the member.
IF YOU HAVE RETURNED TO WORK, put your return-to-work date on your withdrawl card and return it to your local branch office, or call your local branch office and advise of your return date. DO THIS YOURSELF. DO NOT RELY ON SOMEONE ELSE TO DO IT FOR YOU!
